Job summary

Centralnassau is looking for a skilled professional to join the CN Guidance FACT Team in Plainview, NY. The role involves delivering vital services to individuals diagnosed with severe mental illness, intervening where traditional methods falter.

Qualifications include a master's degree in a relevant field and licensure in New York. The successful candidate will engage with recipients through assessments, treatment planning, and coordination of care, ensuring their needs are met 24/7.

Job Definition

Work with the CN Guidance FACT Team to deliver services to individuals diagnosed with severe mental illness whose needs have not been met by traditional service delivery approaches.

Educational Requirements

A master's degree or higher in social work, psychology, rehabilitation counseling, or a related field. Licensure with the NYS Education Department in a specific field is preferred. Limited permits are required if not licensed.

Experience Required

Two years' experience working with the chronically mentally ill (Master's level). Competency in substance abuse, family support, or vocational rehabilitation must be demonstrated by at least one year of training and/or experience in integrated mental health and substance abuse assessment and treatment.

Duties and Responsibilities
  1. In absence of Team Leader, direct and supervise staff activities, lead team organizational and service planning meetings, and provide direction to staff regarding individual cases.
  2. Perform routine team duties and have lead responsibility for integrating dual‑recovery treatment, family support or vocational/employment training with the tasks of other team members.
  3. Responsible for providing assessment and service planning, quality assurance and improvement processes, utilization review, incident reporting, and coordination between ACT and other systems.
  4. Engage in performing assessments, case management, crisis intervention, and discharge planning.
  5. Maintain case records and complete documentation on time and in compliance with federal, state, and agency regulations.
  6. Provide direct telehealth, face‑to‑face and/or collateral services to recipients either in the FACT Office or in the community in places and situations where problems arise.
  7. Be available on an as‑needed basis 24 hours a day, 7 days a week, including responsibility for periodic pager coverage.
  8. Assist recipients in developing appropriate expectations from treatment.
  9. Strengthen the ability of the recipient to understand and utilize therapy and find resources after treatment has ended through proper discharge planning and termination process.
  10. Exchange information with other agencies that benefits the recipient, or ensure compliance with regulations and coordination of services.
  11. Utilize and understand the value of supervision, which increases practice skills, promotes better services for the recipient, and preserves accountability to the Agency.
  12. Support the use of data in clinical supervision to inform decision‑making.
  13. Identify and implement quality improvement trainings and activities as part of professional development goals.
  14. Cultivate a culture of inclusion for all employees that respects their individual strengths, views and experiences.
  15. Adhere to all safety protocols and procedures to ensure a safe working environment for all employees.
  16. Show proficiency with virtual collaboration tools or managing remote teams if applicable.
  17. Perform other duties as assigned.
