Job summary

CommonSpirit Health’s California Hospital Medical Center is seeking a Family Services Coordinator to conduct comprehensive family assessments, develop individualized family plans, and deliver in-home and on-site case management to boost family stability and support early childhood education.

You will manage a caseload of 35-40 families, providing family support, parenting education, and social services including referrals, advocacy, and services brokering while collaborating with a flexible team

Qualifications

• Bachelor's degree in child development, social work, psychology, human development, or a related field.
• Experience working with low-income families and diverse cultural backgrounds.
• Driver's License: CA; Auto Insurance.
• Basic Life Support - CPR and First Aid within 90 days.
• Fire Safety within 90 days.

Responsibilities

• Complete comprehensive family assessments across health care, education, mental health, development, housing, and more.
• Develop individualized family plans promoting self-sufficiency.
• Support caregivers through home visits and center-based contacts to create safe environments.
• Facilitate group socialization experiences, parent trainings, and conferences.
• Identify and refer for mental health and other needs, providing advocacy and guidance.
• Collaborate with community agencies and coordinate joint case conferences.
