Job summary

Gather Health delivers primary care for older adults through in-center visits, telehealth, and home-based care designed to treat the whole person. You’ll work in a social primary care model that prioritizes time, team support, and holistic outcomes.

The role centers on leading a care team to coordinate medical and social needs and to provide longitudinal, relationship-driven care in state-of-the-art centers, via telehealth, and in patients' homes.

We're a Social Primary Care practice serving clinically and socially complex older adults in underserved Boston communities. Our model gives you the time, the team, and the tools to actually treat the whole person not just their chart.

Panel size is capped at 750 patients. Full stop.

• Medical Assistants, Patient Benefits Partners, and Practice Operations Partners handling the administrative and clinical support so you can focus on care
