Gather Health in Massachusetts is recruiting a physician to join our Social Primary Care model serving older adults in Boston. You will lead clinical care across clinics, telehealth, and in-home visits, coordinating with a multidisciplinary team.

We emphasize time, team, and tools to treat the whole person, with a shift to value-based care and outcomes over volume. Administrative time supports documentation and care coordination.

A typical week

• Clinic and telehealth visits with your panel (no double-booking, no 10-minute slots)
• Care team huddles with Guides and Medical Technician Partners
• Remote administrative time for documentation, care coordination, and team communication
• Home-based care coordination supported by your MTP team (you direct; they deploy)
