Primary Care Physician - Cleveland, OH

COMPENSATION: Salary $250k+ Incentive Bonus + Signing Bonus up to $50k

We are a physician-led and mission-driven primary care organization, currently one of the most successful full-provider models, transforming care for the neediest populations. To achieve our vision and fulfill our mission, we seek the best primary care providers and clinical leaders who are seeking purpose and personal growth to join our team. Transportation is provided to patients at no charge.

Role:

1. Compensation: Base $250k
2. Bonus: Up to $20k in year 1, increasing as a partner
3. Partnership: No buy-in; typically within 12-18 months; 65% become partners after 1 year
4. Patient Load: Approximately 18 patients per day
5. Hours: Monday - Friday, 7:30 am - 5:00 pm (last patient scheduled at 4:15 pm)
6. Work/Life Balance: Emphasized
7. Patient Panel: Max 450 patients
8. Administrative time included in schedule
9. No nights or weekends
10. 100% outpatient care
11. Support Team: Dedicated MA, scheduler, and referral specialist
12. Patient Population: Seniors with 3-5 chronic diseases
13. Care Model: Value-Based Care
