1. Base salary plus production bonus and retention bonus every six months

3. Four-day work week with six weeks of PTO and six holidays
4. Average of 20 patients per day in the clinic
5. Approximately 40 deliveries annually per provider
6. $200,000 sign-on bonus and $12,000 relocation allowance
