Responsibilities

• Utilizes best practices to develop and implement plans and monitor outcomes for patients referred for Chronic Illnesses
• Coordinates activities with Primary Care Physicians and Specialists
• Develops and presents educational programs
• Promotes activities to improve overall health and wellbeing of patients
• Fosters a team environment and manages requests for time off
• Reviews reports and participates in Quality Assurance processes
• Makes recommendations for service development and improvement
• Performs medical evaluations and follow-up care as delegated by supervising physician
