Central Ohio Primary Care seeks an experienced, full-time NP/PA to join Granville Pike Family Physicians in Lancaster, OH. The role focuses on comprehensive primary care, preventive medicine, and chronic disease management in an outpatient setting.

Ideal candidates hold an Ohio NP/PA license with FNP/PA certification and have strong clinical and communication skills to work collaboratively in a multidisciplinary team using EPIC EHR.

Qualifications

• Master’s degree in nursing or physician studies from an accredited program.
• Current Ohio NP/PA licensure and national FNP/PA certification.
• Prefer 1 year outpatient practice experience post-graduation.
• Strong clinical, diagnostic, and treatment skills; excellent communication.

Responsibilities

• Perform comprehensive assessments and exams for patients of all ages.
• Deliver full primary care services including well visits, vaccines, and preventive care.
• Create and adjust treatment plans for chronic diseases; manage medications.
• Educate patients and families on health maintenance and disease prevention.
• Coordinate care with physicians, APPs, nurses, and specialists; assist with referrals.
• Document encounters accurately in EPIC per regulatory standards.
