Qualifications

• Master's or Doctorate in nursing (MSN or DNP) from an accredited NP program.
• Active Ohio APRN-CNP license in good standing.
• National NP board certification (AANP or ANCC).
• Active DEA registration.
• Proven experience in primary care patient assessment, diagnosis, and treatment.
• Strong expertise in chronic disease management and preventive care.

Responsibilities

• Conduct comprehensive patient assessments, including health histories and physical examinations.
• Diagnose and treat acute and chronic illnesses in accordance with clinical guidelines and scope of practice.
• Develop, implement, and monitor individualized treatment and chronic disease management plans.
• Provide preventive care services, including immunizations, screenings, and health maintenance counseling.
• Prescribe medications and order diagnostic tests, labs, and imaging as clinically indicated.
• Document patient encounters accurately and thoroughly in the EHR system.
• Educate patients and families on disease prevention, self-management, and healthy lifestyle choices.
• Coordinate care with physicians, specialists, and other members of the interdisciplinary team.
• Follow up on test results, referrals, and care transitions to ensure continuity of care.
• Maintain compliance with state regulations, clinical protocols, and organizational policies.
