Practice Details

• Seeking a BE/BC Family Medicine or Internal Medicine physician to join our Senior Health team.
• Full-time and part-time positions available.
• Fellowship training in Geriatric Medicine not required; Family Medicine or Internal Medicine candidates with a strong interest/experience in Geriatrics are welcome to apply.
• Join an experienced team of Physician and Advanced Practice Providers supported by clinic support staff: 1 RN and 1 Patient Service Rep.
• EPIC EMR system used.
• Responsibilities include nursing home admissions and rounding.
• Opportunity for Medical Directorship of three Nursing Homes for additional stipend.
• Average patients per day: 10.
• Call schedule: 5 days per month, including weekends, handled by phone.
