Job summary

MountainOne is seeking a Family Partner to provide support and advocacy for families working with the Wraparound team. This role involves collaborating with service providers to help families meet their goals and engaging parents in addressing their children’s needs.

The ideal candidate has experience in caregiving for youth with special needs, excellent communication skills, and a flexible work approach to accommodate family schedules. A Bachelor’s degree in a relevant field is preferred.

Qualifications

• Experience as a caregiver of a youth with special needs, preferably mental health needs.
• Bachelor’s degree in a relevant field with one year of experience preferred.
• High school diploma or GED with two years of relevant experience may be substituted.

Responsibilities

• Coordinate and collaborate with providers regarding the Care Plan.
• Engage the caregiver in activities addressing the youth’s Care Plan goals.
• Complete all necessary paperwork in a timely manner.
