Job summary

Child & Family Services, Inc. is seeking a Family Partner to provide advocacy and support for parents and caregivers.

The role includes facilitating caregiver peer groups, connecting families with community resources, and supporting youth treatment planning. You will work under the Senior Family Partner and Program Director, engaging families with diverse cultural needs, attending school meetings, and coordinating with systems of care to promote positive outcomes for youth.

Qualifications

• Experienced adult who is a parent/caregiver of a youth with Serious Emotional Disturbance (SED)
• Ability to work collaboratively with a team and strong interpersonal skills
• Willingness to work evenings/weekends when required
• Understanding systems of care and engagement with families and caregivers

Responsibilities

• Assist families in understanding CSA services and roles within the program
• Engage caregivers with home and community resources to meet youth treatment plan goals
• Coach caregivers to access resources and coordinate with treatment providers
• Participate in ICP meetings to set clear goals and develop a family-centered vision
• Support families by modeling how to access community resources
• Maintain regular in-person contact with guardians and youths as agreed
• Create a comprehensive family assessment highlighting strengths, needs, and cultural identities
• Deliver strength-based, culturally and linguistically appropriate services under supervision
• Promote linkage with outpatient providers and attend meetings related to youth behavioral health needs
• Attend school meetings and participate in developing school plans/IEPs and engage with courts/state agencies
• Monitor readiness for transition and satisfaction with CSA services
• Participate in systems of care committee meetings
• Perform other duties as required
