Job summary

Child and Family Services, Inc. is seeking a Family Partner to support parents and caregivers and to facilitate peer groups within the Community Service Agency. You will advocate for families, connect them to home and community resources, and help plan individualized care alongside the youth's ICP.

The role emphasizes cultural and linguistic sensitivity, collaboration with the Senior Family Partner and Program Director, and regular in-person contact with families to strengthen outcomes.

Qualifications

• Experience as a Family Partner or caregiver in a youth support setting.
• Strong understanding of systems of care and collaboration.
• Ability to work evenings and weekends when required.

Responsibilities

• Assist the family in understanding the scope and services of the CSA and the family’s roles and responsibilities in the program.
• Engage the parent/caregiver in home and community resources designed to address goals on the youth’s treatment plan.
• Teach parent/caregiver how to network and secure community resources and treatment providers.
• Develop goals with the ICP and support a family vision for the youth’s treatment plan.
• Model and educate families on accessing community resources.
• Maintain regular in-person contact with guardians/youth as agreed.
• Create a comprehensive narrative assessment of the family, highlighting strengths, needs and cultural identities.
• Deliver culturally and linguistically appropriate services under supervision.
• Promote linkage with outpatient providers and attend related meetings when requested.
• Attend school meetings and participate in IEP planning and other system meetings.
• Monitor readiness for transition and satisfaction with CSA services.
• Participate in systems of care committee meetings.
• Perform other duties as required.
