Responsibilities

• Educate patients on preventive care and chronic disease management.
• Coordinate care with other clinicians for continuity.
• Maintain accurate EMR documentation and charting.
• Support scheduling and clinical administrative tasks.
• Maintain a clean, safe clinical environment.
