Job summary
Harbor Health Services Inc. seeks a mission-driven Family Nurse Practitioner to join our Primary Care Department at the Harbor Community Health Center in Hyannis, MA. This full-time role includes evening shifts and Saturday rotation to meet clinic needs.

Qualifications
- Completed accredited Nurse Practitioner program in Family Practice.
- Current MA NP License, Family Practitioner, Board Certified, DEA, MCSR and BLS.
- Computer literate (MS Office) and EMR experience (EPIC/OCHIN highly desired).
- 2+ years NP experience required; bilingual preferred (English/Portuguese/Spanish).
- Experience with a community health center setting preferred.
Responsibilities
- Serves as a primary care provider for a panel of patients.
- Organizes and manages care of primary care patients including visits, communication, referrals, and patient education.
- Provides routine health maintenance and well visits with age-appropriate screenings.
- Manages chronic illnesses and acute illnesses with appropriate follow-up.
- Participates in on-call system and evening/weekend clinic schedule.
- Documents care in EMR with accurate progress notes and labs.
