Job summary

Terros Health is hiring a full-time Family Nurse Practitioner for its Oak Recovery Health Center in Phoenix, AZ. The role focuses on primary care and integrated mental health services, conducting exams, diagnosing and treating illnesses, and coordinating with other providers.

Requirements include AZ NP license, MSN/DNP with FNP, BLS, DEA and ANCC board certification, and an Arizona driver’s license. In-office, day shift with potential Spanish/other language differential pay.

Qualifications

Arizona NP license required.
MSN or DNP with Family Nurse Practitioner focus.
BLS certification required.
DEA license and ANCC board certification required.
Arizona driver’s license and driving record per policy.
FingerPrint clearance card or ability to obtain within 7 working days.
Drug screen, TB test and background check required.
Proficient with electronic health records and MS Office.

Responsibilities

conducts physicals/check-ups, evaluations, diagnostics, treatment and prevention.
orders lab tests and prescribes/monitors medications.
educates patients about preventative care and treatment plans.
provides leadership within the clinic and may supervise staff or students.
manages referrals for specialty and behavioral health care.
ensures 85% of work time is direct patient services.
maintains patient files per agency guidelines and state regulations.
