Hope + Health Direct Primary Care in Leander, TX is seeking a Family Nurse Practitioner for a part-time/PRN role to provide comprehensive primary care for patients across the lifespan. Qualifications

• Active, unrestricted Nurse Practitioner license in Texas.
• Board certification through AANP, ANCC, or applicable national certifying organization.
• Strong clinical judgment and communication skills.
• Comfort working independently seeing patients in-office, virtually and using various technology.
• Minimum of 3 years+ of experience in primary care, urgent care, or emergency medicine.
• Experience caring for patients across the lifespan.

Responsibilities

What You'll Do

As a Family Nurse Practitioner at Hope + Health, you will provide comprehensive primary care to patients across the lifespan, including:

• Performing comprehensive physical examinations and health assessments
• Diagnosing and treating acute and chronic medical conditions
• Managing ongoing preventative and primary care needs
• Ordering, reviewing, and interpreting laboratory and diagnostic testing
• Developing individualized, evidence-based treatment plans
• Prescribing medications within your scope of practice and Texas regulations
• Providing patient education and counseling on preventative health and lifestyle
• Coordinating referrals and collaborating with specialists when needed
• Using telemedicine and secure messaging to improve patient access and continuity of care
• Maintaining accurate, timely, and thorough medical records
• Building long-term relationships with patients and their families
• Contributing ideas and helping us continually improve the way we deliver care

What We're Looking For

Required:
