Job summary

One Medical’s Wicker Park clinic in Chicago, IL seeks an Advanced Practitioner to manage a diverse patient panel and provide in‑office, testing center, and telehealth care. You’ll participate in weekly Clinical Rounds and collaborate with both in‑office and virtual teams to deliver high‑quality primary care.

• In the past 5 years, practiced as an Advanced Practitioner in outpatient primary care.
• Experience seeing patients of all ages (0+) in primary care setting.

Responsibilities

• Managing a patient panel with a broad array of patient needs; conducting a mix of acute, chronic, and well visits.
• Treating patients in-office or in testing centers as well as conducting occasional tele-health visits.
• Ongoing learning during weekly Clinical Rounds and through other modalities.
• Collaborating with in-office teammates via daily huddles, and with virtual clinical teams.
• Utilizing your clinical training and opportunities to perform in-office procedures.
• Willing to obtain additional state licensure and credentialing for One Medical virtual primary care in additional states.

What you’ll be working on:

• Managing a patient panel with a broad array of patient needs; conducting a mix of acute, chronic, and well visits

Education, licenses, and experiences required for this role:

• Completed an accredited FNP or PA program with a national certification
• In the past 5 years, practiced as an Advanced Practitioner for at least:
• 2 years in an outpatient primary care setting seeing patients of all ages (0+), OR 1 year in an outpatient primary care setting seeing patients of all ages (0+), coupled with either a 1 year primary care fellowship or 1+ year in an urgent care setting seeing patients of all ages (0+)
• State licensed in IL, obtained by your One Medical start date

This is a full-time role based at our Wicker Park clinic in Chicago, IL.

The base salary range for this role is $126,700 - $151,500 based on a full‑time schedule. Final determination of starting pay may vary based on factors such as practice experience and patient care schedule.
