HealthBar Careers is seeking a Family Nurse Practitioner (NP or CNS) in Grand Rapids for a 32-hour workweek (0.8 FTE) with benefits eligibility. The role emphasizes work-life balance, meaningful patient relationships, and whole-person care within an employer-sponsored clinic.

The clinician will focus on prevention, wellness, and proactive care, outside traditional insurance-driven volume expectations, with autonomy and collaboration in a supportive team.

Qualifications

• Master's degree in Nursing from an accredited program.
• Active, unrestricted Michigan RN license.
• Active, unrestricted Nurse Practitioner certification.
• Active Michigan NP Specialty Certification.
• BLS certification (AHA or Red Cross) within 90 days of hire.
• Must be authorized to work in the U.S. without employer sponsorship.

Responsibilities

• Educate patients on prevention, self-management, and treatment of acute and chronic conditions.
• Provide proactive, relationship-based care focused on prevention and long-term health outcomes.
• Coordinate follow-up care and continuity of care services.
• Counsel patients on medications, supplements, lifestyle interventions, and interactions.
• Order, perform, review, and interpret diagnostic testing.
• Develop individualized treatment plans based on health needs and goals.
