Position Highlights:

• Four-day workweek (4x10s)
• Monday Friday only | No nights or weekends | Minimal call
• 18 22 patients per day
• Supportive team of MAs, front office, and leadership support
• Integrated care model with access to behavioral health, case management, PharmD support, imaging, diabetic education, and dietitian/social work services

Position Requirements:

• Family Nurse Practitioner (FNP) certification
• Eligible for Colorado licensure
• DEA registration with prescriptive authority
• 1+ years of family medicine experience required
