Job summary

Premise Health Inc. seeks a Family Nurse Practitioner to join our Health Center in Schofield, WI, providing care to client employees and their dependents. The schedule includes Tue–Fri and varied hours, with newborn-to-adult patient care.

The role requires NP licensure, DEA for prescribing, and BLS/ACLS as applicable. You will perform exams, procedures, care planning, and patient education in a collaborative team environment.

Qualifications

• Master’s Degree in Nursing required or equivalent.
• National certification (ANCC or AANP) required.
• State RN license and NP licensure required.
• Federal DEA license (matching state of practice) required.
• Prescriptive authority as applicable; NY ROPES registration if required.
• ACLS and BLS certifications may be required depending on contract scope.

Responsibilities

• Conducts physical exams and preventive health measures per guidelines.
• Performs injections, suturing, wound care, and infection management.
• Develops patient care plans, records progress, and ensures continuity of care.
• Educates patients on treatment plans, health maintenance, and development.
• Orders and interprets diagnostic tests to assess clinical conditions.
• Collaborates with physicians and healthcare teams for comprehensive care.
• Prescribes or recommends treatments, including medications and therapies.
• Refers patients to specialists or physicians as needed.
• Maintains patient health records with accurate documentation.
• Educates and coaches nursing staff on best practices.
• Performs other duties as assigned.

Education

Master’s Degree in Science of Nursing

Current national certification (ANCC or AANP)

As a Full Time Family Nurse Practitioner, you'll provide care to client employees and their dependents in our Health Center located in Schofield, WI.

Schedule

• Mon - OFF; Tue 8:00 am - 5:00 pm; Wed 8:00 am - 6:00 pm; Thu 7:00 am - 5:00 pm; Fri 7:00 am - 4:00 pm
• Ability to See Newborn and up

What You’ll Do

What You’ll Bring

• Maintains state RN licenses/certificates to practice
• Maintains Nurse Practitioner licenses/certificates to practice
• Maintains State Prescriptive Authority (if applicable)
• Federal DEA license (matching each state of practice as applicable)
• State Narcotic/Controlled Substance license if required in your state (Alabama, Connecticut, DC, Delaware, Hawaii, Idaho, Illinois, Indiana, Iowa, Louisiana, Maryland, Massachusetts, Michigan, Nevada, New Jersey, New Mexico, Oklahoma, Rhode Island, South Carolina, South Dakota, Utah, Wyoming, Puerto Rico) (New York ROPES registration)
• Current national certification (ANCC or AANP required)
• Master’s Degree in Science of Nursing (if Master Degree in Science certificate does not state Nursing on it then a copy of transcripts is required)
• Current certification with hands-on training in AHA, ARC or equivalent Basic Life Support for health care providers is required; Advanced Cardiac Life Support may also be required based on contract scope of services
• At least three years of clinical experience as a Nurse Practitioner in an ambulatory care, occupational health, family practice or emergency department setting
