We are seeking a Full-timeFamily Nurse Practitioner for our South Bronx Health Center located at 94-98 Manhattan Avenue, Brooklyn, NY 11206 .
Reporting to the Medical Director, this individual will be responsible for providing evidence-based Primary Care to our patients. This individual is an integral of the collaborative care team which includes nursing staff and other services such as Health Education, Nutrition, Social Work and Behavioral Health.
The ideal candidate will have a valid Family Nurse Practitioner N.Y.S. License to deliver medical care and at least two (2) year of post-graduate/residency experience delivering care in a Primary Care/Ambulatory Care setting.
