Qualifications

• Active license & current board certification required.
• Independent practice experience preferred, where applicable per State regulation.
• CPR/BLS certification required at time of start date.
• Federal DEA number preferred; may be required for full prescribing ability.

Responsibilities

• Practice relationship-based medicine at the top of your license.
• Provide prevention, chronic disease management and health education to patients.
• Work autonomously with strong resources and a patients-first culture.
