Job summary

Gándara Mental Health Center, Inc. is hiring a Family Partner in Massachusetts to work with families receiving clinical services. The role requires lived experience as a caregiver for youth with special needs and supports navigating child-serving systems to access services and follow care plans.

You will mentor families, assist in transitioning to services, and collaborate with a clinical team to empower caregivers 24/7 in a diverse community setting.

Qualifications

• Minimum of two (2) years’ experience working with children/adolescents/transitioning youth.
• Experience as a caregiver of youth with special needs, preferably mental health issues.
• Experience in navigating child and family-serving systems and teaching family members.
• Strong communication, administrative and organizational capabilities.
• Able to work with diverse populations and community agencies.

Responsibilities

• Provide education and support during treatment and attend care coordination meetings.
• Educate caregivers on navigating child-serving systems and resources.
• Support parents/caregivers in accessing peer and community support.
• Provide care coordination via calls, meetings, and warm handoffs.
• Identify community resources to sustain treatment goals.
• Collaborate with a multi-disciplinary team to empower families.
