Job summary

Thrive Homes is seeking compassionate individuals to become Family Model Providers in Tennessee. You will open your heart and home to one or two individuals with disabilities, offering room and board and direct personal care, including cooking, bathing, cleaning, medication assistance, and accompanying to appointments.

This is a full-time/contract role in an in-person, live-in setting with strong emphasis on person-centered supports.

Qualifications

• Minimum of 1 year of experience working with individuals with intellectual disabilities.
• Clear criminal background check is a must.
• Access to a safe and accessible home or apartment with a dedicated bedroom for the individuals, especially if wheelchair accessibility is required.
• Reliable vehicle and valid auto insurance and driver's license for transportation.
• Completion of all required training, including Relias, Medication Administration Certification, CPR and First Aid.
• Access to a personal computer with internet connectivity for documentation purposes.
• High school graduate or equivalent education preferred.

Responsibilities

• Open your heart and home by offering room and board to one or two individuals with disabilities.
• Provide person-centered direct personal care, including ADLs such as cooking, bathing, cleaning, and medication administration assistance.
• Accompany individuals to medical appointments and support PCSP goals and outcomes.

Key Responsibilities:

As a Family Model Provider, you will open your heart and home by offering room and board to one or two individuals with disabilities. You will be responsible for providing person-centered direct personal care, including activities of daily living such as cooking, bathing, cleaning, medication administration assistance, and accompanying individuals to medical appointments. Your support will play a crucial role in helping the individuals thrive, achieve independence, and work towards their personal goals and outcomes outlined in their PCSP (Person-Centered Support Plan).
