Family Medicine Physician Job with Fairview Health Services in Apple Valley, MN 55124

Fairview Health Services is seeking a Family Medicine physician to join our primary care practice at M Health Fairview Clinics – Apple Valley. Join our busy practice with over 20 years of community presence.

Practice Details:

1. 11-provider primary care practice with more than 5 specialists onsite.
2. Average of 18-22 patient encounters per day.
3. 34 patient contact hours weekly, with a low call frequency of 1:10, primarily phone consultation with nurse triage.
4. Position: 1.0 FTE (Full Time), with a flexible schedule.
5. Part of a Pioneer ACO, utilizing a multidisciplinary team approach.
6. Fully integrated Electronic Medical Record system (EPIC).

5. Meet credentialing and privileges requirements if applicable.
