Qualifications

• A minimum of five years of clinical experience in Family Medicine, with a strong emphasis on chronic disease management, EMR proficiency, behavioral health collaboration, and preventive care delivery.
• Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and other healthcare professionals.
