Job summary

Ascension Health is seeking an experienced physician to provide outpatient primary care within an Internal Medicine residency program in Milwaukee, WI. The clinic emphasizes high-quality, patient-centered care for Medicaid and Medicare populations, with a focus on coordinated transitions and continuous improvement.

You will join an established team and practice autonomously within a clinic setting, contributing to a trusted community health anchor managed by Ascension.

Qualifications

• Licensure/Certification/Registration required before hire.
• Advanced Life Support preferred (ALA/Red Cross accepted).

Responsibilities

• Outpatient primary care within an Internal Medicine residency program.
• Join an established team with RN triage and 1 independent APP.
• Deliver high-value, patient-centered care for Medicaid/Medicare populations.
• Coordinate care transitions, including consultations and discharge planning.
• Lead or participate in continuous quality improvement initiatives.

This job location is currently located in a Health Professional Shortage Areas (HPSAs) and/or Medically Underserved Areas/Populations (MUA/P) Shortage Designation Type.

• Outpatient primary care practice embedded in an Internal Medicine residency program


• Join an established team of 1 independent APP


• RN triage team in place


• Primarily Medicaid and Medicare


• Laboratory services available


• Long-standing clinic serving as a healthcare anchor in the community


• Meaningful, high-value care for patients with significant social determinants of health


• Deliver comprehensive, patient-centered primary care, including high-quality diagnostic and treatment services, to improve the health and well-being of the community.


• Coordinate seamless care transitions for patients, managing specialized consultations, inpatient admissions, and collaborating with case management for successful discharge planning.\


• Lead or actively participate in continuous quality improvement initiatives to enhance the efficiency and cost-effectiveness of care delivery within the clinic and the wider system.


• Maintain complete and timely patient documentation and electronic medical records to ensure compliance, accurate records, and support the financial integrity of the practice.


• Ability to practice autonomously and demonstrate comfort managing a solo outpatient primary care panel within a clinic environment

What minimum requirements you’ll need

Licensure / Certification / Registration:

• Physician MD/DO credentialed from the Wisconsin Medical Examining Board obtained prior to hire date or job transfer date required.


• Advanced Life Support preferred. American Heart Association or American Red Cross accepted.

Education:

• Doctor of Medicine (MD) or Doctor of Osteopathy (DO) required.
