Job summary

Ascension’s Capitol & Lily Clinic in Brookfield, WI is seeking a physician to provide comprehensive, patient-centered primary care in an established outpatient setting. You will work with a team including 3 Internal Medicine physicians, 1 APP, and regional RN triage support, with onsite lab and x-ray.

Responsibilities

• Deliver comprehensive, patient-centered primary care, including high-quality diagnostic and treatment services.
• Coordinate seamless care transitions for patients, including consultations, inpatient admissions, and discharge planning with case management.
• Lead or participate in quality improvement initiatives to improve efficiency and cost-effectiveness of care delivery.
• Maintain complete and timely patient documentation and EMR accuracy to support financial integrity of the practice.
• Ability to practice autonomously and manage a solo outpatient primary care panel within a clinic setting.

This job location is not currently located in a Health Professional Shortage Areas (HPSAs) and/or Medically Underserved Areas/Populations (MUA/P) Shortage Designation Type.

How you’ll make an impact in this role

• Join an established team of 3 Internal Medicine physicians, 1 Internal Medicine APP, 2 Family Medicine physicians, and a PRN Internal Medicine PA
• Staffing includes 1:1 MA support, a daytime regional RN triage team, and an evening RN triage team managing after-hours calls.
• Onsite services include lab and x-ray.
• Specialists rotating into the clinic include Podiatry, Neurology, Sleep Medicine, and Orthopedics, with a behavioral health clinic located upstairs.
• Deliver comprehensive, patient-centered primary care, including high-quality diagnostic and treatment services, to improve the health and well-being of the community.
• Coordinate seamless care transitions for patients, managing specialized consultations, inpatient admissions, and collaborating with case management for successful discharge planning.
• Lead or actively participate in continuous quality improvement initiatives to enhance the efficiency and cost-effectiveness of care delivery within the clinic and the wider system.
• Maintain complete and timely patient documentation and electronic medical records to ensure compliance, accurate records, and support the financial integrity of the practice.
• Ability to practice autonomously and demonstrate comfort managing a solo outpatient primary care panel within a clinic environment.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration:

• Physician MD/DO credentialed from the Wisconsin Medical Examining Board obtained prior to hire date or job transfer date required.
• Advanced Life Support preferred. American Heart Association or American Red Cross accepted.
