Job summary

LeoForce, based in Lebanon, PA, is seeking a dedicated Family Medicine Physician to join our healthcare team. This permanent, full-time role offers comprehensive care for patients of all ages.

You will work with a multidisciplinary team to deliver high-quality, evidence-based medicine and population health initiatives. The ideal candidate has a Doctor of Medicine or Doctor of Osteopathic Medicine degree, board certification or eligibility in Family Medicine, and a valid state license with DEA.

Qualifications

• MD or DO degree from an accredited medical school.
• Board certification or eligibility in Family Medicine.
• Active state medical license and DEA license.
• Minimum of 2 years of experience in Evidence-Based Medicine, Chronic Disease Management, Preventive Care, Care Coordination, Medication Management/Prescribing, and Population Health.

Responsibilities

• Provide comprehensive and continuous care to patients and their families.
• Utilize evidence-based medicine to diagnose and treat a variety of acute and chronic conditions.
• Develop and implement patient management plans, including preventive care.
• Coordinate patient care with other healthcare professionals and refer patients to specialists when necessary.
• Prescribe and manage medications in accordance with patient needs and regulatory guidelines.
• Monitor the health status of patients with chronic conditions and adjust treatment plans as necessary.
• Educate patients and their families on disease prevention and health maintenance.
• Participate in population health initiatives to improve the health of the communities we serve.
• Adhere to all relevant medical laws, regulations, and standards.
