Job summary

Australia-Employment seeks a Family Medicine Physician to join our mission-driven community health center in Lebanon, PA. You will provide comprehensive primary care across ages and work with an interdisciplinary team to improve community health outcomes.

• Board Certified or Board Eligible in Family Medicine.

Responsibilities

• Provide comprehensive primary care to patients across the lifespan.
• Diagnose and treat acute and chronic medical conditions using evidence-based medicine.
• Develop individualized treatment plans that incorporate preventive and wellness-focused care.
• Manage chronic conditions and monitor patient progress, adjusting treatment plans as needed.
• Prescribe and manage medications in accordance with patient needs and regulatory requirements.
• Coordinate care with other healthcare professionals and make specialist referrals when appropriate.
• Educate patients and families on disease prevention, health maintenance, and treatment plans.
• Build strong, ongoing relationships with patients and their families through continuity of care.
• Participate in population health initiatives designed to improve health outcomes within the community.
• Maintain compliance with applicable medical laws, regulations, and clinical standards.
• Collaborate with an interdisciplinary team to ensure coordinated, high-quality patient care.

Experience and Licensure:

• MD or DO degree from an accredited medical school.
• Board Certified or Board Eligible in Family Medicine.
• 2+ years of experience in family medicine or a related primary care setting.
