Ascension is seeking a Family Medicine physician to join FCA Family Health & Wellness Clinic in Kokomo, IN. This outpatient, employee position offers a stable, mission-driven environment within a nonprofit Catholic health system.

Join a collegial team with a structured call rotation, diagnostic resources on-site, and a focus on preventive care, patient education, and long-term wellness for employees and their dependents.

Qualifications

• Must be licensed in Indiana before hire or transfer date.
• Advanced Life Support preferred.
• Board Certification in Family Medicine or Board Eligible.

Schedule: Full Time

Productivity Incentives: Salaried position

CME allowance $3000 + 40 hours dedicated CME time (based on FTE status)

This job location is not currently located in a Health Professional Shortage Areas (HPSAs) and/or Medically Underserved Areas/Populations (MUA/P) Shortage Designation Type.

How you’ll make an impact in this role

This is an exceptional opportunity for a compassionate Family Medicine Physician to join an employed position within a busy, patient focused practice, supporting the FCA employees and their dependents. Preventative Medicine/Lifestyle Medicine preferred but not required.

• Join a well established, collegial teams
• Call structure: 1 week every 6 weeks
• Practice setting: Outpatient
• Employed position within a nonprofit health system
• Board Certified or Board Eligible in Family Medicine
• Eligible for state medical licensure and DEA registration
• Deliver comprehensive, high-quality care that prioritizes building strong, enduring patient-provider relationships for optimal long-term health outcomes.
• Optimize patient well-being by rigorously assessing current health risks and deploying proactive strategies for disease prevention, early detection, and health education.
• Lead collaborative efforts across the multidisciplinary care team to ensure seamless coordination and a holistic approach throughout the patient journey.
• Ensure urgent patient needs are met with accessibility by offering same-day appointments for acute illness and unexpected medical concerns.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration:

• Physician MD/DO credentialed from the Indiana Medical Licensing Board obtained prior to hire date or job transfer date required.
• Advanced Life Support preferred. American Heart Association or American Red Cross accepted.

Education

• Doctor of Medicine (MD) or Doctor of Osteopathy (DO) required.
