South Coast Community Services is seeking a Full-time family services coordinator to support the Wraparound program. The role involves locating and reconnecting family members, coordinating with Social Workers and Probation Officers, and representing SCCS at planning meetings and community events.

The position requires strong communication, documentation skills, and reliability in transportation for local travel between office locations and community sites.

Competenze

• Must be proficient in Microsoft Word, Excel and Microsoft Outlook.
• Must be able to communicate clearly, both verbally and in writing.
• Must be able to author and produce emails, letters, reports and other documents in a professional manner.
• Ability to interact professionally and effectively with all levels of employees within the organization.
• Must be detail oriented and have the ability to complete documentation in a timely manner.
• Excellent time management, organizational and follow through skills.

Mansioni

• Manage Family Finding database on N: drive.
• Document all services provided in client's case record.
• Discuss Family Finding Services with the referred client/family at Client Family Team (CFT). Obtain legal guardian signature on forms.
• Meets with the client/family to identify potential family members to reconnect with.
• Coordinate Family Finding with SW/PO and FAC.
• Collaborates with SW/PO and FAC to process status of contact efforts with client/family.
