Responsibilities

• Establish partnerships with Head Start families and community resources.
• Support outreach, recruitment, transitions in PFCE content areas.
• Connect families to resources supporting school readiness and family wellness.
• Provide case management and referrals, and document services provided.
• Coordinate with staff to share home information and track child performance.
