Job summary

The Oregon Department of Human Services is seeking a Family Coach (Human Services Case Manager) to assess family needs, determine eligibility for program benefits, and complete applications for support. You will work with clients to develop case plans, monitor progress, and connect them to community resources.

Responsibilities include crisis planning for domestic violence survivors, collaboration with partners, and flexible hours to meet program needs in a store-front office setting.

Qualifications

  • Bachelor’s degree in a Behavioral Science, Social Science, or a closely related field.
  • Bachelor’s degree in any field and one year of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing).
  • An Associates degree in a Behavioral Science, Social Science or a closely related field AND two years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing).
  • Three years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing).

Responsibilities

  • Conduct interviews to determine family needs and eligibility, and complete program applications for program benefits and, if eligible, issues appropriate support.
  • Engage clients in family evaluation of strengths and jointly develop a case plan and identify the steps the family will take towards self-sufficiency and meeting their basic needs.
  • Monitor progress, needs, and makes necessary modifications to the case plan.
  • Initiate and participate in case staffing, employment readiness, life skills training, and supports job search as needed.
  • Assist with crisis and safety planning for individuals experiencing domestic violence.
  • Explore alternate resources and link clients to community services and may also assist with supporting resources and services within the community.
