Job summary

The Oregon Department of Human Services seeks a Family Coach (Human Services Case Manager) to assess family needs, complete program applications, and connect clients with community resources, emphasizing equity and trauma-informed practices in a Warm Springs setting.

You will develop case plans, monitor progress, support employment readiness, and assist with crisis planning for domestic violence survivors, coordinating with district offices and community partners in a fast-paced environment.

Qualifications

Bachelor's degree in Behavioral Science, Social Science, or a closely related field.
Bachelor's degree in any field + one year of human services related experience.
Associate degree in a Behavioral Science, Social Science or related field and two years of human services related experience.
Three years of human services related experience.

Responsibilities

Conduct interviews to determine family needs and eligibility, and complete program applications for program benefits and, if eligible, issues appropriate support.
Engage clients in family evaluation of strengths and jointly develop a case plan and identify steps toward self-sufficiency and basic needs.
Monitor progress, needs, and make modifications to the case plan.
Initiate and participate in case staffing, employment readiness, life skills training, and supports job search as needed.
Assist with crisis and safety planning for individuals experiencing domestic violence.
Explore alternate resources and link clients to community services and supports.
