Live Well Homecare is seeking a Family Centered Treatment Practitioner to deliver structured, time-limited in-home therapy to at-risk children and families, with the aim of preventing out-of-home placements.

You will conduct assessments, develop treatment plans, provide multi-hour sessions in family homes and community settings, coordinate with child welfare and other relevant systems, manage crises, and document services accurately.

Qualifications

• Experience in in-home therapy.
• Strong assessment and planning abilities.
• Crisis management skills.
• Adherence to FCT model standards.
