Qualifications

• Must complete FCT training and achieve certification.

Responsibilities

• Complete and maintain all relevant FCT training and work toward Provider Certification.
• Provide FCT to families and guide them through four phases with measurable goals.
• Maintain program fidelity documents in a timely manner.
• Provide at least two multi-hour face-to-face sessions weekly with each family.
• Maintain a flexible schedule including evenings/weekends as needed.
• Participate in weekly provider team meetings and serve as facilitator/recorder on a rotating basis.
• Participate in weekly case staffings and multi-disciplinary team meetings.
• Provide thorough documentation including fidelity documentation and progress notes.
