Penfield Children's Center in Milwaukee is seeking a Service Coordinator for the Birth to Three program. You will manage a caseload, connect families with essential services, and support child development through family-centered coaching.

Collaborating with therapists, social workers, and teachers, you will help families navigate resources, plan transitions, and ensure timely access to services while maintaining thorough documentation.

Qualificações

• Bachelor's degree in social work, early childhood, human services, or related field.
• Two years of experience providing case management, family support, or early intervention services.
• Experience with IFSP/IEP/service plans and multidisciplinary teams.
• Experience documenting across multiple systems and billing/compliance requirements.

Responsabilidades

• Support families in Birth to Three through caseload management and timely access to services.
• Coordinate IFSP development, reviews, and referrals with the team.
• Schedule home visits and provide monthly family contact.
• Assist families in identifying resources and navigating services.
• Document services accurately in electronic systems.
