Job summary

Curative Care in Milwaukee seeks a Family Service Coordinator to provide in-home case management for children aged 0–3 with developmental delays. You will conduct screenings, facilitate eligibility, and help develop care plans.

You will connect families with community resources, teach advocacy, and assist with discharge services, while maintaining strong documentation, organization, and customer service skills to support diverse families.

Qualifications

• Knowledge of the local service delivery system and resources.
• Bachelor’s Degree in a human service related field as described.
• One year of supervised experience with families with special needs preferred.
• Valid Wisconsin driver's license, reliable transportation and car insurance required.
• Experience with children aged 0–3 and a love of working with them.

Family Service Coordinators provide in-home case management/service coordination for children ages zero to three who evidence a developmental delay or other atypical development. Service Coordinators conduct screenings, facilitate eligibility determination, organize and participate in evaluations and develop plans for care. Service Coordinators link families with community resources, teach families how to be effective advocates for their children, and assist families in arranging for services their children may need at the time of discharge. Service Coordinators demonstrate excellent documentation, organization and customer service skills.

Skills and Qualifications

The Family Service Coordinator must have:

• Knowledge of the local service delivery system, the target group’s needs, the need for integrated services, and the resources available or needing to be developed
• A Bachelor’s Degree in a human service related field (Human service related fields are those that include coursework in areas such as human development, psychology, social services or similar areas)
• One (1) year of supervised experience working with families with special needs preferred
• A valid Wisconsin driver’s license, reliable transportation and car insurance are required
• Experience with children in the 0-3 age range and a love of working with them.
