Family Service Coordinators provide in-home case management/service coordination for children ages zero to three who evidence a developmental delay or other atypical development. Service Coordinators conduct screenings, facilitate eligibility determination, organize and participate in evaluations and develop plans for care. Service Coordinators link families with community resources, teach families how to be effective advocates for their children, and assist families in arranging for services their children may need at the time of discharge. Service Coordinators demonstrate excellent documentation, organization and customer service skills.
