Job summary

Mentor Behavioral Health, part of Sevita, delivers Early Intervention, ABA, and behavioral health services to children and families. You will perform diagnostic evaluations, craft treatment plans, and coordinate with referral agencies to support each individual’s goals.

The role emphasizes building trust with families, collaborating with teams, and providing intensive home-based therapy to strengthen therapeutic experience within a mission-driven environment.

Qualifications

• LPC, MFT or LSW required.
• Two years related field experience.
• Specialized training provided.
• Exceptional communication skills with the ability to establish trust and rapport quickly.

Responsibilities

• Complete diagnostic evaluations.
• Develop treatment plans based on assessment of strengths, needs, collaboration with referral agencies, and collaboration with the individual served.
• Provide activity and participation-based interventions and approaches developed from training materials, experience, individual and/or families’ successes.
• Make referrals to services and resources on behalf of individuals and/or families served.
• Ensure documentation and records are kept confidential, organized, and submitted to supervisor in a timely manner.
• Participate in team meetings with peers and collateral teams as needed.
• Support peer development by addressing areas of concern for professional development and team performance issues.
• Provide an aftercare plan for each family/individual served.
• Build and maintain positive relationships with community partners from referring/collateral agencies. Guide families in their interactions with referring agencies and other community organizations as needed.
