Job summary

Mentor Behavioral Health, a part of Sevita, seeks an Outpatient Therapist to provide therapeutic interventions to youth, families and adults. Services are delivered in homes, communities, the office and virtually to meet individual needs.

Join a mission-driven team with a full compensation/benefits package, $60K per year and a $4K bonus, plus quarterly incentives, 401(k) with match, PTO, flexible schedule, and free CEUs and supervision for licensure.

Qualifications

• Bachelor's, LSW with 3+ years' experience with children and families.
• Master's in human services and licensure (LPC or MFT) required.

Responsibilities

• Conduct diagnostic evaluations.
• Provide therapy, family support, and crisis interventions to individuals, families and/or groups.
• Provide mental health consultation to stakeholders and treatment team members.
• Develop individualized treatment plans with an interdisciplinary team.
• Coordinate discharge planning.
• Maintain timely health record documentation.
• Attend staff meetings and supervision to review cases.
• Provide 25 service delivery hours per week.

• Develop individualized treatment plans in coordination with an interdisciplinary team

• Attend regular staff meetings, clinical meetings and one-on-one sessions with direct supervisor to process current status of cases.
• Provide 25 service delivery hours per week.
