Job summary

Aviva Family and Children’s Services in Los Angeles is seeking an Intensive Care Facilitator to coordinate, schedule and lead Child and Family Team (CFT) meetings with clients in home, school or community settings.

You will document Safety Plans, Outcome Measures, CFT Matrix and progress notes, collaborate with DMH, DCFS and other partners, and ensure culturally competent, family-driven care.

Qualifications

• Bachelor’s degree in Social Work, Sociology, Psychology or related field.
• 1 year of relevant experience.
• Bilingual (English/Spanish) required.
• Experience in crisis management preferred.
• Knowledge of DMH services preferred.

Responsibilities

• Coordinate CFT meetings to develop, guide, and plan service delivery.
• Schedule meetings and manage logistics including location and materials.
• Collaborate with cross-system partners to coordinate care.
• Complete documentation: Safety Plan, Outcome Measures, Matrix, progress notes.
• Monitor progress and adjust interventions as needed.
• Maintain confidentiality per HIPAA and regulations.
