Job summary

Virginia Birth Injury Fund is seeking a Case Specialist to support admitted participants and families through holistic, family-centered case management. You will build trusted relationships, conduct assessments, coordinate program benefits, and provide ongoing support while working with an interdisciplinary team and traveling to participant homes.

The role emphasizes trauma-informed approaches, de-escalation, and advocacy to improve outcomes for participants with complex medical and social needs.

Qualifications

  • Bachelor’s degree in Social Work, Nursing, Human Services, Public Health, Counseling or a related field.
  • Minimum of 5 years of progressively responsible experience in case management or related field.
  • Experience with children and young adults with complex medical needs strongly preferred.
  • Experience conducting home visits and coordinating community-based services preferred.
  • Current licensure or eligibility in Virginia (e.g., LCSW, LMSW, RN, LPN, OTR/L) preferred.
  • Valid Virginia driver's license and access to reliable transportation.
  • Excellent interpersonal, written, and verbal communication skills.
  • Strong judgment and escalation skills.

Responsibilities

  • Manage an assigned caseload by providing ongoing assessment, care coordination, proactive follow-up, and support to participants and their families.
  • Develop individualized service and support plans in collaboration with participants, families, and interdisciplinary team members.
  • Coordinate services across multiple Program benefits and community resources to promote participant well-being and quality of life.
  • Monitor participant progress and adjust support strategies as needs evolve.
  • Apply family-centered and trauma-informed principles in all interactions with participants, families, providers, and community partners.
  • Document assessments, interventions, home visits, care plans, referrals, and follow-up activities.
