Job summary

HeartShare St Vincent’s Services is seeking a Family Case Planner in a child welfare preventive services program in New York. The role provides direct casework counseling to families with children under 18 at risk for placement, using the Solution Based Casework model.

You will perform intake, crisis intervention, home visits and develop service plans with supervision from the Program Director. Benefits include a robust package and professional development opportunities.

Qualifications

• Bachelor's degree required or Master's degree preferred.
• Two years experience in Child Welfare or related Human Service field.

Responsibilities

• Direct Services to clients via casework, individual, family and group counseling as indicated by individual case assessments.
• Assist in Intake and Crisis Intervention.
• Conduct home visits and attend client related meetings as needed.
• Develop client assessments and service plans following the outlined models.
• Monitor and assist in service provision to all program clients.
• Maintain current case records and service plans (FASP) for families served utilizing Connections and PROMIS systems.
• Participate in weekly supervision, case conferences and Family Team Conferences as directed by Supervisor and Program Director.
• Provide parent education and other support services utilizing group format.
• Participate in training to develop Social Work skills as required by Children’s Services and HeartShare St Vincent’s Services.
• Assist in day to day operations as required by the Program Director.
