The State of Indiana is seeking a dedicated Family Case Manager in Shoals, Indiana to protect children and support families. You will work in a public service environment, handling investigations, case planning, and coordination with adoption and foster care teams.

Qualificações

• Bachelor’s degree or equivalent in human services.
• Two years of relevant human services experience can substitute.
• Five years of relevant experience may be considered in lieu of degree.
• Knowledge of social work laws and foster care policies.

Responsabilidades

• Investigate reported incidents of child abuse, neglect, or dependency and determine substantiation for disposition.
• Assess risk to the child and determine need for protective measures or removal.
• Perform needs assessments to determine treatment options for families and children.
• Prepare reports and briefs for community child protection teams and develop informal adjustment plans.
• Develop case plans to promote self-sufficiency and permanent homes for children.
• Monitor program/service providers for alignment with the case plan.
• Accept on-call responsibilities to investigate allegations as needed.
• Conduct home studies and recommend placements for adoption.
• Visit children in placements to coordinate reunification or independent living.
• Maintain data to help management evaluate trends in family needs.
