Job summary

State of Indiana’s Department of Child Services seeks a Family Case Manager in Fort Wayne serving Allen County and surrounding areas. You will protect children, reunify families when possible, and pursue permanent homes or independent living for youth.

Duties include home visits, case planning, coordination with providers, and court testimony. You will contribute to safeguarding Hoosier families, maintain case documentation, and collaborate with community resources to ensure outcomes aligned

Qualifications

• Associate degree with at least two years of human services experience.
• 30 college credit hours from an accredited institution with at least four years of human services experience.
• At least five years of human services experience.

Responsibilities

• Investigate reported child abuse or neglect and determine disposition recommendations for Juvenile Court or County Director.
• Assess risk to the child and determine need for protective action.
• Perform needs assessments to determine treatment options for families and children.
• Prepare reports and case files; testify at court hearings; develop informal adjustment plans.
• Develop case plans to help families achieve self-sufficiency and safe, permanent homes.
• Discharge/transition planning to maintain family stability.
• Monitor service providers to ensure alignment with the case plan.
• Accept on-call responsibilities as needed for investigations.
• Conduct home studies and family histories; recommend adoption placements.
• Visit children in out-of-home placements to coordinate reunification or independent living.

Education
