Job summary

State of Indiana in South Bend is hiring a Family Case Manager to protect children and support families in St. Joseph County.

You will conduct home visits, develop case plans, coordinate with community resources, testify in court, and prepare documentation for files and reporting. Responsibilities include investigating abuse/neglect, assessing risk, creating discharge plans, and ensuring a safe permanent home for children while maintaining compliance with state laws.

Qualifications

• Requires associate degree or 30 college credits with 2–4 years of human services experience
• Minimum 5 years of human services experience may be considered in lieu of degree
• Advanced knowledge of social work principles, casework, sociology, and counseling

Responsibilities

• Investigate reported child abuse/neglect and determine disposition recommendations
• Assess risk level and potential removal to protect the child
• Develop case plans and coordinate with community resources
• Attend and testify at court hearings and prepare documentation for files
• Develop discharge/transition plans for families to attain self-sufficiency
• Monitor service providers to ensure alignment with the case plan
• Maintain data to evaluate trends in child and family needs
