State of Indiana is hiring a Family Case Manager in Indiana to support families and protect children from abuse and neglect. You will work to reunify families when possible or secure a permanent home for children.

Qualifications

• Associate degree with a minimum of two years of human services experience.
• 30 college credit hours with at least four years of human services experience.
• Five or more years of human services experience.
• Advanced knowledge of social work, casework and counseling principles.
• Thorough knowledge of state and federal child protection laws and standards.
• Ability to coordinate across social services programs.
• Ability to work with diverse families and individuals.

Responsabilités

• Investigate reported child abuse or neglect and determine substantiation and disposition.
• Assess risk to children and determine protective actions as needed.
• Perform needs assessments and develop treatment options.
• Prepare reports and briefing documents for court or community teams.
• Develop case plans to achieve safe, permanent homes or independent living.
• Coordinate discharge/transition plans to promote self-sufficiency.
• Monitor service providers for compliance with the case plan.
• Accept on-call responsibilities for investigations as needed.
• Conduct home studies and advise on adoption placement.
• Visit children in placement to support reunification or independence.
• Maintain data to track trends and outcomes for management.
