Qualifications

• Ability to manage multiple cases and deadlines effectively.

Responsibilities

• Conduct intake assessments to identify family strengths, needs, and risks.

• Regularly review and update case plans based on progress and changing needs.
• Connect families to healthcare, behavioral health, housing, and social services.
• Coordinate services across multiple agencies and providers.
